Form about Music
Hi readers! The school is finishing, but we have few work to do. The last that I have made is this form about music. To create this form I have used google docs, and there I have used their form maker. It is very usefull, and very easy to use, I recommend you this tool. Here you have the form to complete it. I hope you would like it.

Just a couple of comments about your form: firstly, the question about the groups they like is a multiple choice question, so you don't allo readers to choose more than one. I think a "checkbox" exercise would have been better. Secondly, the scale grid is usually organised the other way round: 5 is usually better than 1. It doesn't really matter, but it may cause problems with people who tend not to read instructions.
